Responsibilities:

 

  • Conduct comprehensive health assessments, including medical history, physical exams, and diagnostic tests, to evaluate patients' health status.
  • Diagnose and treat common acute illnesses, injuries, and chronic medical conditions in collaboration with supervising physicians and other healthcare providers.
  • Prescribe medications, order and interpret diagnostic tests, and provide appropriate treatments and interventions based on evidence-based practice guidelines.
  • Develop and implement individualized treatment plans in collaboration with patients and their families, emphasizing health promotion, lifestyle modifications, and preventive healthcare measures.
  • Educate patients and families about health maintenance, disease prevention, medication adherence, and self-care management strategies to optimize health outcomes.
  • Coordinate care with specialists, allied health professionals, and community resources to ensure continuity of care and comprehensive healthcare services for patients.
  • Maintain accurate and up-to-date medical records, documentation, and billing codes in compliance with regulatory standards and organizational policies.

Qualifications:

  • Master's degree in Nursing from an accredited Nurse Practitioner program.
  • Current certification as a Family Nurse Practitioner (FNP) by a nationally recognized certifying body (e.g., AANP, ANCC).
  • Active and unrestricted license to practice as a Nurse Practitioner in the state of California.
  • Minimum of 2 years of clinical experience as a Nurse Practitioner, preferably in primary care or family practice settings.
  • Strong clinical assessment and diagnostic skills, with the ability to formulate differential diagnoses and develop appropriate treatment plans.
  • Excellent communication and interpersonal skills, with the ability to establish rapport and collaborate effectively with patients, families, and interdisciplinary healthcare teams.
  • Proficiency in electronic health record (EHR) systems and computer literacy.
  • Commitment to providing culturally competent, patient-centered care and promoting health equity for diverse populations.
