Responsibilities:
- Conduct comprehensive health assessments, including medical history, physical exams, and diagnostic tests, to evaluate patients' health status.
- Diagnose and treat common acute illnesses, injuries, and chronic medical conditions in collaboration with supervising physicians and other healthcare providers.
- Prescribe medications, order and interpret diagnostic tests, and provide appropriate treatments and interventions based on evidence-based practice guidelines.
- Develop and implement individualized treatment plans in collaboration with patients and their families, emphasizing health promotion, lifestyle modifications, and preventive healthcare measures.
- Educate patients and families about health maintenance, disease prevention, medication adherence, and self-care management strategies to optimize health outcomes.
- Coordinate care with specialists, allied health professionals, and community resources to ensure continuity of care and comprehensive healthcare services for patients.
- Maintain accurate and up-to-date medical records, documentation, and billing codes in compliance with regulatory standards and organizational policies.
Qualifications:
- Master's degree in Nursing from an accredited Nurse Practitioner program.
- Current certification as a Family Nurse Practitioner (FNP) by a nationally recognized certifying body (e.g., AANP, ANCC).
- Active and unrestricted license to practice as a Nurse Practitioner in the state of California.
- Minimum of 2 years of clinical experience as a Nurse Practitioner, preferably in primary care or family practice settings.
- Strong clinical assessment and diagnostic skills, with the ability to formulate differential diagnoses and develop appropriate treatment plans.
- Excellent communication and interpersonal skills, with the ability to establish rapport and collaborate effectively with patients, families, and interdisciplinary healthcare teams.
- Proficiency in electronic health record (EHR) systems and computer literacy.
