Requirements:

• Active and unrestricted CA license (MD, PA, or NP)
• Board Certified (Family Medicine, Internal Medicine, or FNP)
• DEA license (Schedule II–V), CURES registration, NPI
• American Heart Association BLS Certification
• Minimum 3 years’ primary care experience
